GERGA Booking Form

  • This form has been designed to be used for the duration of the national lock down, until such time as all students return to normal campus activities.
  • This is not a booking form for GERGA’s physical facilities, but a form to request GERGA’s support team immediately prior to and during assessments.
  • It remains the department’s responsibility to check and accept all invitations on the GERGA’s online calendars.
  • Please check GERGA’s calendar if the GERGA online assessment support team are available, before placing your request.

This form needs to be completed and sent to gergabookings@sun.ac.za. Right-click and use the “Save Link As…” option. Opening up the form using the Chrome or Firefox default PDF viewer will not work and allow the form to be completed. The form needs to be opened in Adobe Reader or Adobe DC Reader in order for the form to work.
